I am Nicole Greenberg, PhD. I provide psychoeducational evaluations to assist in understanding your child and obtaining optimal outcomes in their educational environment. I have received extensive training in neurodevelopmental disorders through graduate education at the University of Alabama and a clinical internship at University of Alabama at Birmingham and Children's of Alabama, where I completed the LEND (Leadership Education in Neurodevelopmental and Other Related Disorders) program. My goal is to help you and your family navigate your child's strengths and weaknesses, and for you to obtain the necessary tools to aid in your child's educational and personal development. Policies and Procedures: - At this time, I do not accept insurance. However, upon request, I may provide you with a receipt that you can submit to your insurance. - A free 10-15 minute initial consultation can be scheduled through the "Request Appointment" tool or by contacting me directly by phone (850-270-7294) or email (nicole@nicolegreenbergphd.com). This consultation will help determine the testing services that best fit your needs. Additionally, we will review the clinic procedures and policies. - Afterwards, upon notice that you have decided to proceed with testing, intake forms will be sent to you through the client portal to be completed prior to your first appointment. This appointment may also be scheduled through the client portal. - A deposit of $200 is due prior to scheduling the first evaluation appointment. That deposit will be credited towards your final balance; however, it will be forfeited if you fail to appear for a scheduled evaluation and do not provide notice of cancellation at least 24 hours in advance. - All evaluations, excluding intelligence testing, involve a 1-hour caregiver clinical interview (in-person or via telehealth). Certain evaluations may also require a release of information form so that the the child's teacher may complete behavioral rating forms. - Testing sessions with your child last between 1.5 hours (intelligence testing) to up to 4 hours (comprehensive testing), based upon the type of evaluation requested. - A 30-60 minute feedback session is scheduled with caregivers to discuss results of the evaluation and to provide a copy of the final written report. This will be scheduled 3-5 weeks after the final testing session or after all the necessary paperwork has been received (e.g., teacher rating forms). - Signed consent from a legal guardian is required for testing.
