If you are a first time client, please use the request appointment button to schedule an initial evaluation. Once the appointment time has been requested, you will receive a follow up email with instructions to complete our practice documents. Unfortunately, we do not provide evaluations or treatment for custody, competency, disability, medication, emotional support animals or services mandated by Children and Youth Services.
