Introduction

Daniel Moldwin is a Board-Certified Psychiatric Nurse Practitioner who completed his training at Duke University. He currently provides outpatient medication management to clients conveniently via telemedicine. Building strong trusting relationships, providing education, and engaging in client-centered care are all important aspects of Daniel’s treatment approach. His goal is to optimize each person’s quality of life, helping people to feel happier and more fulfilled. He is proud to provide clients across New York State with easily accessible, affordable, quality mental health care. His goal is to create a safe, trusting, and engaging environment while ensuring that clients receive top quality, evidence informed care. This entails spending a great deal of time assessing, diagnosing, planning, implementing, and evaluating. Treatment plans are tailored to individual needs and not developed in a cookie-cutter fashion. Daniel will present all pharmacologic options that are available and will work collaboratively with the client to determine which medication is the best fit. By the end of each visit, the client will feel they are heard and will fully understand their diagnosis and treatment plan.
